Prescription Refill and Food Order Request Form

Please use the form below to request your food item or prescription refill. We also carry all of your dog or cat’s “toiletries”… ear cleaners, nail trimmers, shampoos, toothpaste, and more! If there is something special you are looking for just ask, we would be happy to see if we can get it for you. Simply fill out the applicable boxes, add any special comments or information you need us to know, such as whether you’d like pills cut for you and we will place your order for you. Please allow 2 business days for order processing. Once your order has arrived at the clinic, we will call or email you with a confirmation that it is ready to be picked up. Please Note: Some prescription medications will require an examination of your pet prior to re-filling. This ensures that your pet’s health status has not changed and provides further confirmation that the medication and dosing are appropriate for your pet’s current condition. If there are any concerns regarding your request one of our staff members will contact you directly.
